American Board of Dental Hypnosis

 

Requirements for Application
  1. An essay on hypnosis and dentistry of approximately 800 to 1,200 words
  2. Evidence of having taken intermediate and/or advanced hypnosis courses that would meet ASCH approval.
  3. A statement as to how, and how long, he/she has used hypnosis as part of his/her practice, or conducted research on hypnosis.
  4. Two case histories.
  5. The above submitted with a check for $100.00 to Dr. Dov Glazer, 3525 Prytania St. #312, New Orleans, LA 70115-3566.
  6. The examination will cover a general knowledge of the following areas of hypnosis: definitions, history, theory, misconceptions, phenomena, applications, language, dangers, adverse reactions, and hypnotizability scales.
  7. One month prior to the examination the applicant will submit a $150.00 fee for the examination.  If the applicant passes the examination, the $150.00 fee will be applied as an entry fee to the Board of Hypnosis in Dentistry.

Reprint(s) of any article (or articles) concerning hypnosis which the applicant may have published.
